How it works
Build your questions
Open Client forms. Add questions, conditional follow-ups, photo requests and signature fields in the Questions tab.
Choose where and when
In Where it’s used, select appointments, classes or courses. Ask during booking, after confirmation and before the visit, or by manual request.
Review the responses
Open Responses on the form, or find the submission from the client profile and its booking or course enrolment.
The right questions for each service
Use one form across relevant services, or give a consultation, class or course its own questions. Conditional follow-ups keep unrelated questions out of the way.
Photo requests in the form
Ask clients to upload JPEG, PNG or WebP photos alongside their answers. Photos stay in private storage and are opened through permission-checked links.
Control when clients fill it in
Collect required information during booking, give clients time to complete it before their visit, or send a request yourself. Choose whether an eligible response can be reused or a fresh one is needed for every booking.
Each participant has their own response
Class forms follow the participant, including guardian bookings. Course forms belong to each enrolled participant and cover the course, so answers stay with the person attending.
Responses where you need them
Client forms is the place to build forms and review submissions. The client profile brings together their history, while a booking or course enrolment gives you the response for that visit. Access depends on the staff member’s role and eligible clients; front-desk form status does not grant access to private answers.
Clients use their private booking portal to see pending requests and completed status. If you add a form to a service with existing bookings, review and send requests to those clients explicitly. Changing the service assignment does not silently email earlier bookings.
Staff-written observations belong in Client notes, a separate area. Forms collect what the client tells you; notes record your own follow-up.
Common questions
Are these forms only for appointments?
No. Client forms, photo requests and timing controls work with appointments, classes and courses. Choose the relevant services in Where it’s used.
Can I ask clients to complete a fresh form?
Yes. Set the form’s completion and renewal rules. Reuse depends on a verified client identity, the matching form version and your policy; you can require a new response for every booking.
Where can I see booking responses?
Open the form’s Responses tab, the client’s profile, or the related booking or course enrolment. The staff member must have permission to read the response.
What photos can clients upload?
JPEG, PNG and WebP are supported, up to 10 photos, 10 MB each and 50 MB in total per submission. If a phone photo cannot be converted, save it in a supported format before uploading.
Does Junocal provide the wording for my waiver?
You write and approve the questions and consent wording for your service. Junocal provides the form, signature and response tools; it does not supply a clinically validated intake template or determine whether your waiver is legally sufficient.
